First IVF Cycle Guide: From Day 1 to Embryo Transfer

The decision to start your first IVF cycle is a monumental step. While the process involves several clinical stages, understanding the timeline can transform a sense of "the unknown" into a structured, manageable journey.

Preparation and Day 1
Before the cycle begins, we focus on preparing for your first IVF cycle by assessing your ovarian reserve. This involves blood tests (AMH) and an ultrasound to count antral follicles.

Day 1 of your cycle is the first day of full menstrual flow. You will notify your clinical coordinator, and usually, on Day 2 or 3, you will have a baseline ultrasound to ensure the ovaries are "quiet" and ready to begin stimulation. This means that there is no cystic follicle in the ovaries that could prevent the other follicles from responding as well as possible.

Ovarian Stimulation

Over the next 10-12 days, you will self-administer hormonal medications, under the guidance of your doctor regarding the correct dose and the appropriate medication , personalized to your needs.

This achieves the development of the already existing follicles that begin in your cycle, instead of the development of only one follicle, as happens in a natural cycle.

  • Monitoring: You will visit the clinic 3-4 times for scans and blood tests(if needed).
  • The Trigger Shot: Once the follicles reach the optimal size, you receive a "trigger" injection of hCG or a GnRH agonist to finalize egg maturation.

Egg Collection (The Procedure)

Exactly 34-36 hours after the trigger shot, the egg collection takes place. This is a short, 15-20 minute procedure performed under light sedation. A small needle, guided by ultrasound, enters the follicles, aspirates the follicular fluid, and it is then given to the embryologists for processing.

You’ll rest in our recovery suite for a few hours and can head home shortly after.

Fertilization and Embryo Development

The retrieved eggs meet the sperm in the lab via conventional IVF or ICSI. This is where the magic of science takes over. We monitor the stages of embryo development closely:

  • Day 1: Confirmation of fertilization.
  • Day 3: The cleavage stage (6–8 cells).
  • Day 5/6: The Blastocyst stage.

The Embryo Transfer

The transfer is the simplest part of the clinical process—much like a regular smear test. No sedation is required. Using a fine catheter, the doctor places the embryo into the uterine cavity. It is a moment of great hope, and we encourage you to remain relaxed and positive.

The " Wait"

The final stage is the 9 days wait before a pregnancy test. While it is tempting to use home kits early, we recommend waiting for the scheduled blood test (beta-hCG) for the most accurate result. During this time, you will continue with progesterone support to help the uterine lining remain receptive.
